Philadelphia Freedoms/Advanta Corporation

2004 Philadelphia High School Sportsmanship Award

One male and one female Philadelphia-area high school student who is nominated for this award and best meets the criteria, as determined by representatives of the Philadelphia Freedoms and Advanta Corporation (the sponsors of the award), will receive the following award: (a) attend and sit on the bench with the members of the WTT Philadelphia Freedoms at one home match; (b) a Wilson® tennis racquet; and (c) an autographed WTT Philadelphia Freedoms team jersey. The 3 components of this award must be accepted as is – they are not redeemable for cash and no substitutions will not be made.

Candidate Criteria for the 2004 Sportsmanship Award

Candidates Must:

Ø1. Currently be on a Philadelphia area high school varsity tennis

Ø2. Play tennis on a Philadelphia area tennis team

Ø3. Be a good role model on and off the tennis court

Ø4. Have a minimum GPA of 2.5/4.0 (C ) (or, if another grading system

is used by the high school, the equivalent of a 2.5/4.0 GPA)

Please write and attach a 50-100 word essay describing in detail the candidate’s

sportsmanship, motivation, attitude, strengths and honors

and why and how the candidate meets the criteria outlined above.

All nomination forms must be received by the Philadelphia Freedoms’

office by 5:00 p.m. on Saturday, May 1, 2004.

  • You must agree to these rules in order to nominate a candidate.
  • This award program is void outside the city of Philadelphia, Pennsylvania and is subject to any applicable federal, state and local laws and regulations. Void where prohibited by law.
  • No purchase or payment of money is necessary to nominate a candidate. There is no cash or exchange value for the award or any award component and the award is not transferable. Nominations for the award will be accepted during the period commencing at 9:00 a.m. on or about March 1, 2004 and continuing through 5:00 p.m. on May 1, 2004.
  • Advanta Corporation and the Philadelphia Freedoms (“we”), and each of their parents, subsidiaries, and affiliates and their respective employees and immediate family members are not eligible to nominate a candidate nor may they be nominated as a candidate.
  • The odds of your candidate winning depends upon the number of eligible students nominated in their same gender. All decisions of the Philadelphia Freedoms and Advanta Corporation are final.
  • We do not warrant that the provision of the award will not interfere with any other award or scholarship for which the candidate may apply. Please check with the high school, district and/or state before nominating the candidate for this award or if you have any concern regarding the candidate’s eligibility for other awards and/or scholarships.
  • We are not liable (to you or any third party) or responsible for, and will not accept, any postage due, late, mis-delivered, incomplete, destroyed, mutilated, faxed, or emailed nominations of any kind, nor will we be liable to you or any third party for any problems or damages related to your accessing the internet, visiting the websites where this form is located, or downloading or printing this form and rules. We are not liable to you or any third party for any claims or damages if your nominated candidate is not chosen for the award.
  • All nomination forms become the property of the sponsors and will not be returned.
  • Any and all taxes and related reporting are the sole responsibility of award recipients and their parents or guardians.
  • The sponsors of this award, Philadelphia Freedoms and Advanta Corporation, assume no responsibility or liability for damages, losses, or injuries resulting from the nomination of a candidate and/or the candidates award, acceptance, and/or use of any and all parts of the award. The sponsors reserve the right, based on their sole and absolute discretion to interpret compliance with the letter and intent of these rules, to disqualify any nomination deemed to have violated these rules or to have been filled out incorrectly or incompletely at anytime prior to the delivery of the award. Additions or deletions to the rules are subject to change without notice.
  • This award will be governed by the laws of the Commonwealth of Pennsylvania. Any and all legal disputes or claims arising in connection with this award must be brought in a court of competent jurisdiction in the Commonwealth of Pennsylvania.
  • The judges named by the sponsors will select the 2 winners, one male and one female, from all eligible nominations received by Philadelphia Freedoms no later than 5:00 p.m. on May 1, 2004. Each winner will be notified by telephone or email. At that time, they will be asked to sign supporting documentation and a publicity release and the winner’s refusal to do so may result in forfeiture of the award.
  • By submitting a nomination, you accept the terms and conditions stated in these rules, agree to be bound by the decision of the judges, and warrant to the sponsors that the candidate is eligible to participate in this award program and receive an award if chosen.
